Cambridge ESOL course application form completion notes

Important note
No field may be left empty/blank.  If necessary write 'n/a' (not applicable) or 'none', as appropriate.
1.  Choose your course
Choose your course from the drop down menu.
2.  Personal Details
Full name, Address, Landline number, Mobile number, Email address, Date of birth, Nationality and First language.  Please try and provide a landline number, especially if you live outside Turkey.  N.B. Your full name should consist of your first name, followed by your surname (last name), separated by a space, e.g., 'Michael Jackson'.  Please don't write 'Jackson, Michael' or anything like that.  You may include middle names if you wish.
3a.  Do you require a telephone interview?
Telephone interviews are only for applicants who don't live in or around Istanbul.
3b.  Preferred time for interview
Please tell us when you would prefer to have your interview.  For example, if you are not a morning person, select 'weekday afternoons' or 'weekday evenings', or both.
4.  Health
Please tell us about any physical or mental health issues you may have that could potentially affect you on the course.  Everything you say here will be kept in the strictest confidence and nothing you say will affect your chances of being selected for the course.  We ask only so that we are prepared and able to assist you should the need to do so arise.
5.  Education & Qualifications
List all schools, colleges, and universities attended, from secondary school onwards.  Include approximate dates of attendance and list the qualifications you obtained.  If you didn't attend university you may still apply.
6.  Languages
If English is not your first language please provide details of your English education.  Include the name of the institution, approximate dates, relevant exam results and your current level of ability.  Everyone should list languages other than English and their current level of ability.
7a. English language teaching experience
List all prior English teaching experience.  Include the name of the institution, approximate dates, and the type and level of classes taught.  If this amounts to less than 3 years, please also complete the next section.
7b. General work experience
List all work experience over the last five years.  Include the name of the institution, position held, approximate dates and reason for leaving.
8.  References
List two professional referees.  Include their name, the name of the institution/company where they work and their current contact details (address, telephone number and email address).
9.  Additional Information
List any other information that may be relevant to your application.  Include your reasons for doing the course as well as your interests and hobbies.
10.  Written assignment
Write a short essay of between five and eight hundred words.  (The title of the essay is included on the application form).  When writing your essay, please make sure that you:
  • Answer the question.
  • List any and all source material that you refer to in a bibliography section at the end of your essay and quote any passages that are taken directly from an external source.
  • Proof read your essay throroughly.
  • Observe the word count.
Plagiarism is completely unacceptable and will result in automatic disqualification from the application process.
11.  Opt out of the ITI Job List?
Answer 'yes' if you don't want your email address to be added to the list of people who receive an email from us each time a school, university, or company advertises a position on the ITI Job List.  The bulk of jobs advertised on this list are for positions in institutions in Turkey, but you will occasionally receive an email informing you about a position in an institution outside Turkey.
12.  Market research
Please describe how you heard about ITI.  Was it the recommendation of a friend (Word of mouth), the recommendation of a professional institution such as the British Council (Professional referral), an Internet search (Internet), or some other way (Other)?
